Documents to be Submitted to the Internship Committee Before the Internship:

1. Internship Application and Approval Form
2. Institution Information Form where the internship will be done.
3. Copy of Identity Card (for students with Turkish citizenship) / Residence Permit (for foreign students) 
4. Office Registration Certificate (from the most recent year)

Documents to be Submitted to the Internship Committee After the Internship:

1. Template to be used while preparing the internship notebook: Internship Report
2. Employer Internship Evaluation Form
